• Copies of transcripts that have legalized
• Copy of valid ID Card
• Recent color photograph size 4 × 6 cm, 4 (four) pieces are named
• Copy of Certificate of Accreditation Program on graduation from BAN PT
• Filling cover letter (Form has been provided at the time of Job Fair)
